The 5 Whys technique.

The 5 Whys technique is a simple but effective tool for uncovering the root cause of a problem. It involves repeatedly asking "Why?" to get to the heart of an issue. This method, popularized by Toyota as part of its Lean manufacturing approach, can be used in different situations to solve problems effectively.



How the 5 Whys Technique Works


1. Identify the Problem: Clearly define the specific problem you are facing.

2. Ask Why: Ask why the problem occurs and note the answer.

3. Ask Why Again: For each answer, ask why it happens and note the new answer.

4. Repeat: Keep asking "Why?" until you identify the root cause. This usually takes about five iterations, but can be more or fewer depending on the complexity of the problem.

5. Implement Solutions: Once the root cause is identified, develop and implement solutions to address it.

Problem: The project is behind schedule.

1. Why is the project behind schedule?

   - Because the last task took longer than expected.

2. Why did the last task take longer than expected?

   - Because there were unexpected technical issues.

3. Why were there unexpected technical issues?

   - Because the initial design was flawed.

4. Why was the initial design flawed?

   - Because the design specifications were not clearly defined.

5. Why were the design specifications not clearly defined?

   - Because there was inadequate communication between the project team and the client.
